Services

I provide assessment and therapy services for adults and adolescents experiencing a wide range of difficulties. Services are provided at 144 Courthouse Square, Suite 100, Goderich, Ontario.

 Confidentiality

All discussions with me are confidential. No one will have access to the personal information obtained through this service, such as consultation and progress notes without signed consent, except where required by law. Exceptions include when a client presents clear intentions to harm self or others, in cases of child abuse or neglect, or abuse by a regulated health professional, or in the situation where a file is court ordered to be released.

 Fees

Upon signing this document, you understand that the agreed upon fee for therapy services with myself is set at $150 per 50 minute session. Additional fees for reports, letters and documentation for legal purposes will be discussed as needed. For specialized assessments, such as Forensic Risk assessments or ODSP assessments, the total fee is dependent on the time required to complete, and complexity of the case. These fees will be discussed prior to the rendering of services.   

 Cancelled or Missed Appointments

To ensure maximum benefit of therapy, clients should make sessions a priority and should not cancel appointments, except in cases of emergency.  Should you be unable to attend, please use the on-line booking system found at www.claritypsych.ca to cancel your session a minimum of 24 hours in advance of your appointment. A cancellation fee of $50 will be charged should less than 24 hours’ notice be provided.
